Below is a questionnaire template that you might use at your Town Hall Meeting.  Feel free to adapt it to your own needs, by renaming, adjusting, adding or deleting questions and/or adding your logo or slogan at the top.  It was adapted from the Calhoun County Substance Abuse Council Questionnaire and is shared here with their permission.  Please be sure to give credit on any copies you generate.
